College Visit Permission Slip
College Visit Permission Slip - View and print form
Students should call the Admissions Office at the college of their
choice to make an appointment at least two weeks in advance. It is
advisable to also set up an appointment with a professor in the
student’s desired field on the day of the college visit, and possibly
a financial aid officer.
St. Henry District High School strongly encourages parents to attend the campus visit with their child
whenever possible.
The following guidelines must be followed for college visits taken during the school year:
- A student is allowed three visits on or during school days. Students may visit as much as they like on days off or
weekends. After three school-day visits, the student will take an unexcused absence.
- If a visit is during the school day, the counselor must receive at least a one-week notice and the appropriate permission slip allowing the student to make
the visit.
- An official acknowledgement of the visit from the college needs to be given to the school counselor when the student returns the next school day.
- The student must have the college acknowledge the visit on official college stationary - including the date and the student's name.
- Only three students are permitted to go on a college visit on a given day.
- No student will be permitted out of school for a college visit after April 15th.
- The high school administration may waive any of the above requirements in special situations.
