Athletic Forms
Parents/Guardians:
St. Elizabeth Sports Medicine will be offering their annual pre-participation physical examinations for the 2009-2010 school year beginning on June 4th and continuing through July 23rd. Athletes should schedule a private physical throughout the summer, primarily on Tuesdays and Thursdays from 1:15pm - 3:30pm. Physicals will be offered at a $25.00 cost. Physicals will be held at the Edgewood and Florence offices.
Below you will find a letter from St. Elizabeth explaining the physicals, as well as a consent form. You will need to provide the KHSAA physical form which can be downloaded from our website as well the St. Elizabeth consent form. Space is limited for physicals and appointments will be filled on a first come first serve basis.
This KHSAA form must be filled out completely before a student/athlete will be allowed to participate in practices, competitions, games, open gyms, conditioning sessions, etc. Please click on the link and print out the form, have it filled out completely, and bring it into the school office.
